Clinical Guidelines for CPM following Rotator Cuff Repair: Synchronized or Isolated Protocol

چکیده
The primary goal of post-surgical rehabilitation following rotator cuff repair is to control pain, protect repaired tissue during the healing process, restore function, improve range-of-motion, restore strength and prevent a recurrence of symptoms.30 During the initial healing phase following surgery, six weeks of passive range of motion is performed to protect the surgical site.16,17,18,23,25,26,28 Active and active-assisted exercise that result in a muscle contraction are not performed during the initial healing stage in order to protect the integrity of the repair. The amount of protection that is needed for healing is determined by the size or quality of the tear and the type of procedure used for the repair. Protecting tissue does not mean avoiding motion. Passive range of motion is utilized to prevent adhesions, prevent the detrimental effects of immobilization, reduce pain, reduce edema, reduce inflammation and stimulate soft tissue healing.1,3,5,9,12,15,16,18,19,25,26 For a growing number of surgeons continuous passive motion (CPM) has become the Gold Standard for passive motion therapy during this six week period.6,9,12,15,16,28 CPM has demonstrated enhanced tendon healing that is statistically superior to intermittent motion and counters the harmful effects of immobilization.4,7,13,29

A Comparison of Rehabilitation Methods After Arthroscopic Rotator Cuff Repair
CONTEXT Despite the significant attention directed toward optimizing arthroscopic rotator cuff repair, there has been less focus on rehabilitation after rotator cuff repair surgery. OBJECTIVE To determine the effect of different rehabilitation protocols on clinical outcomes by comparing early versus late mobilization approaches and continuous passive mobilization (CPM) versus manual therapy a...
متن کاملCharacteristics of Rotator Cuff Repairs Revised to Shoulder Arthroplasty
Background: Successful repair of a torn rotator cuff may prevent progression to rotator cuff arthropathy. However, previous studies have shown a substantial rate of failure after rotator cuff repair and characteristics of surgicallyrepaired rotator cuffs that go on to shoulder arthroplasty have not been fully elucidated. The purpose of this study wasto determine the patient cha...
متن کاملRehabilitation after Rotator Cuff Repair
BACKGROUND Rotator cuff tears are a very common condition that is often incapacitating. Whether non-surgical or surgical, successful management of rotator cuff disease is dependent on appropriate rehabilitation. If conservative management is insufficient, surgical repair is often indicated. Postsurgical outcomes for patients having had rotator cuff repair can be quite good. A successful outcome...
متن کاملPreliminary Results of a Consecutive Series of Large & Massive Rotator Cuff Tears Treated with Arthroscopic Rotator Cuff Repairs Augmented with Extracellular Matrix
Background: Recurrence rate of rotator cuff tears is still high despite the improvements of surgical techniques, materials used and a better knowledge of the healing process of the rotator cuff tendons. Large to massive rotator cuff tears are particularly associated with a high failure rate, especially in elderly. Augmentation of rotator cuff repairs with extracellular matrix or synthetic patc...

PURPOSE The purpose of this study was to evaluate clinical and anatomic outcomes of patients following transtendon rotator-cuff repair of partial articular supraspinatus tendon avulsion (PASTA) lesions. PATIENTS AND METHODS Patients in the senior author's practice who had isolated PASTA lesions treated by transtendon rotator-cuff repair were included (n=8) and retrospectively reviewed. All pa...
